Additional Payments - Academic Staff:|
This form is to be completed by substantive Academic staff doing extra teaching hours. It should be sent directly to payroll before the relevant academic payroll deadline.
Additional Payments - Professional Support Staff:|
Replaces the Monthly Overtime claim form. NOTE: This form must be printed out on yellow paper.

Flexible Working (Application) Form:
| You can use this form to make an application to work flexibly under the right provided in law to help eligible employees care for their children.

Flexible Working (Acceptance) Form:
|You must write to your member of staff within 14 days following the meeting with your decision. This form can be completed by you when accepting an offer.

Flexible Working (Rejection) Form:
|You must write to your member of staff within 14 days following the meeting with your decision. This form can be completed by you when declining an application to work flexibly.

Flexible Working (Appeal) Form:|
If your application has been refused, you may appeal against your employer's decision. You can use this form to make your appeal.

Flexible Working (Appeal Reply) Form:|
This form is used when replying to an appeal that an application to work flexibly has not been properly considered. This form will be sent to the employee, giving notice of the decision, within 14 days after the meeting to discuss the appeal. If the appeal has been turned down, an explanation on the grounds for refusal is given on this form.

Flexible Working (Extension of Time Limit) Form:
|This form is provided for you to complete when confirming agreement with your employee that you wish to extend a time limit for part of the procedure, from that set out in the regulations.

Flexible Working (Withdrawal) Form:|
This form provides notification to your employer that you wish to withdraw your application to work flexibly. Once you have withdrawn your application, you will not be able to make another application until 12 months from the date that your original application was made.

If you wish to make a claim for interview expenses, please complete and return this form to personnel. NOTE : This form must be printed out on blue paper.

Part-time Lecturers' Claim for Fee Form (F9):|
This form is to be completed by Part-time, Hourly-paid Lecturers, and members of Academic staff on fractional contracts. It should be sent directly to payroll before the relevant academic payroll deadline. NOTE : This form must be printed out on green paper.
Part-time Lecturers' Claim for Fee Form (F11):|
This form is to be completed by Professional support staff doing teaching hours. It should be sent directly to payroll before the relevant academic payroll deadline.

Payroll Deadline Dates Form:
|Please ensure that all claims are made before the relevant payroll deadline date in order to be paid on time.
Pay Spine & Grading Structure|
The new pay spine and grading structure as implemented under the HERA process.
Probationary Review Form:|
Probationary Reviews should be completed by the line manager on 3 and 9 months for Academic staff; 3 and 6 months for Professional support staff. Completed forms should be returnd to Simon Booth in the personnel office.

Self Certification Medical Form:|
Self Certification Medical Forms should be completed by staff who have been off work with an illness between 0.5-7 days. Illnesses lasting more than 7 days require a doctor's certificate.
Staff Expenses Claim Form:|
Expenses claim for all staff i.e. mileage.
Staff Record Form:|
The staff record form is used to update our database when changes are made e.g. change of address, name, emergency contact etc.

Temporary Supply (Request to Fill) Form:|
Line managers requiring temporary staff assistance for up to 4 weeks should complete this form and return to Personnel for approval.
